To register for services, please complete the "Appointment Request" page and be sure to include a mailing address along with your child's name and age. Upon receipt of the request, we will mail out a registration packet for you to fill out and return to CBC, Inc.
In Chicago we are conveniently located at 4801 W. Peterson Ave, Suite 403. Another option unique to CBC, Inc. would be Home Visits (which incur an additional surcharge).
